What documents are required with my CCAP application?

DCFS requires specific documents; certified state birth certificate, recent physical, proof of lead and TB test, and proof of vaccinations.

How do I renew my childcare?

Redetermination papers are due before the end of your contract.  We must have two current consecutive pay stubs and all pages of the Redetermination form mailed to you by CCRS with the barcode.  This includes the page that says, “Keep for your records”.  Please contact CCRS if you did not receive a form in the mail at 1-800-325-5516.

What are the required documents for my child?

What are the sign-in/sign-out and drop off policies?

All children must be signed in and out by an adult over the age of 18 daily.  No child can leave our facility without the adult signature of a person who is on the pick-up list. 

If you are running late in the morning and know you will not make it in by 9:30 you must call Ms. Liz or Ms. Tegan to get approval.   

If your child is not picked up by 5:00 PM and you have not called to notify us you will be late you will be charged a late fee. The fee is $10 per child per 15 minutes. This amount is due when you pick up your child that day.

What is the clothing/diapering policy?

If you have a child in the infant through the Pre-K room, you will need to provide us with at least one change of clothing for each season and spare undergarments for your child.  If the child is still in diapers, you need to make sure they have diapers daily.  The amount of diapers a child uses daily is 4-6.  For a child to move to the Pre-K 4 Room they must be potty-trained.

What is the sickness policy?
No child will be accepted with a fever, vomiting, diarrhea, eye infection, or suspicious rash.   Any contagious conditions will need to be reported to us.  We will advise you the number of days he/she will not be allowed to attend. If your child runs a fever over 100.4 while they are in our care, vomits more than once, or has 3 soft bowel movements they will not be allowed to return for 24 hours or until they are symptom free for 24 hours without medicine.
When are payments due?

Payments are due by the end of each month.

What is the behavior policy?

1st offense – verbal warning for c.hild

2nd offense – talk to the parent

3rd offense – Official write-up

1 day suspension

3 day suspension

5 day suspension

Expulsion

Behavioral consequences may be adjusted depending on severity of the child’s behavior

What clothing is not allowed?

Flip Flops and Crocs are not allowed at ABC Preschool at any time for any age group.  Proper Shoes must be worn by all children daily.

If girls wear dresses, they will need shorts underneath.
